Assumption of Risk and Release
I am the parent or legal guardian of the player named in this registration. I understand that participation in the U District Youth Zone ("the Event") involves physical activity and inherent risks, including the risk of injury. I confirm my child is physically able to participate.
In exchange for my child's participation, I release and agree to hold harmless U District Advocates, its volunteers, officers, and event partners, and the owners and managers of the event venue, from any claims for injury, loss, or damage arising out of my child's participation, except to the extent caused by their gross negligence or willful misconduct.
I authorize event organizers to seek emergency medical care for my child if needed and accept responsibility for any resulting costs.

Photo & Media Release
Photo and Media Consent
Photos and video may be taken throughout the Event for community, promotional, and news purposes, including on social media and the Event website.
YES, by checking the Photo / media release box above in the affirmative, I grant U District Advocates permission to use images and video of my child from the Event, without compensation, in its promotional and community materials.
NO, by checking the Photo / media release box above in the negative, please do not use images or video of my child in promotional materials. I understand my child may still appear incidentally in wide crowd shots of a public event and in video screened during the event.
